My wife and I are going through a separation and are in the process of either a legal seperation or divorce, I have been covered on her insurance. She will no longer cover me on her insurance. I am disabled and have no income, can I apply for AHCCCS with a legal separation?

I have a 2 year old from my previous marriage. Her father moved out of state after we separated and filled for divorce since i did not know of his whereabouts i was given sole custody. I have not changed my last name back to my maiden name because i wanted my daughter to have my same last name. Can i add my last name after her father's last name? maybe hyphenated? what is the process? does her father have to sign? where do i go to start the process?

I have full physical custody and shared legal custody. years ago we agreed he would get the tax write-off for our child 2 yrs in a row, I one. I have an almost 6-year-old son; never married; son's father is on his birth certificate and I receive child support, but no written custody order or anything else. (Does that mean paternity has been established, but I by default have legal custody?) Son's father moved out of state a few years ago; we maintain regular contact and do visits when we're able.
