PP = Parenting Plan CS = Child Support My PP/CS was based on my son living with my ex and being with me on weekends, holidays & school breaks. I agreed to pay her triple CS for 24 months. My PP/CS has yet to be signed off by a judge. My ex has now left my son in my care mon-fri. & attending school in my area. Can I request to rescind the PP /CS agreement based on the fact she is not following the parenting plan submitted? She is of the opinion that since it is essentially a 50/50 parenting plan she is still entitled to the triple CS amount even though she is not following the PP plan.

I changed my name when I was 19 YOA with the Social Security and then got married. I'm now divorced and the box was not checked off to restore my maiden name. Now, I want to get my drivers license in Arizona but, my birth certificate has my birth name and my divorce shows my married name. Now AZ MVD told that there is no legal document when I change my name when I was 19. So they what me to change my name back to my birth name just so I can get a drivers license. Now my California ID show my changed and married name Social Security has me in there system with both names.

I am the father of an 8 year old daughter born out of wedlock. No legal paternity has ever been established, but I've maintained a close relationship with my daughter all along as well as paid child support from day one. I would like to begin the process of obtaining legal joint (if possible) custody of my daughter ahead of fears my ex may try to relocate again (already moved from to CO to AZ). I believe I will need to establish legal paternity which is no problem as I am on the birth certificate and my ex will agree, as well as establishing permenant visitation rights and also the ability to bar her from moving again. Is there a step by step process available online to set this into motion? I am unable to afford an attorney, but might be able to afford a paralegal? Please advise.

A newly licensed dentist removed all of my teeth except the front six. When he removed my bottom teeth, he did a lot of damage - splintered some of the bone, and tore out a lot of tissue. He did not even use my x-rays; instead, he hung up a dental-chart illustration and used that. He used a pliers-like instrument to violently wrench my teeth from my head, all the while sweating and swearing. He failed to anesthetize my lower front teeth and reprimanded me for flinching when he removed them in the absence of novocaine. I am still receiving bills from them. How do I file a small claim?
