You may want to contact the Bankrtupcy Hotline for information on filling out forms. 866-553-0893. The Bankruptcy Courts in Arizona have self-service centers that may be able to assist you. Also, the Bankruptcy Court website has videos that can walk you through the basics of filing different types of Bankruptcy actions.
