How to change the name of my Newborn Child? Is a hearing still required?

I have a 2 year old daughter from a previous relationship, which the dad (for the most part) is not in the picture. I just recently got married, so my name is now the same as my husbands. We also want to change my daughters last name to be the same. How do I go about doing that? Is it possible to do so without the paternal fathers consent, when she has my maiden name currently and the father is not on the birth certificate?

I live in Nevada, but my father lives in Arizona. His wife claims that she has a restraining order against my wife and I, prohibiting us from contacting my father. I have never been served or even contacted by any law enforcement entity in Nevada or Arizona. Is there any definitive way to to determine if there is an order in place?

Was approved last year for Ahcccs Medical in July and our income went up $28 in January this year, SS disiblilty increase. I could not make the change online so I called and someone over the phone, end of Dec, being proactive, do not want to be w/o coverage, they did a new app. I received a note on Feb. 20th saying that it was denied, I was $12 over the income limit, although the new income limits online for Feb. 1 have gone up and I fit in those limits. I called and explained, but no. Applied on marketplace and a yes for reg plan. Can Accchs cancel and leave me w/o any ins. till plan starts?

my house was raided for identity theft and baby daddy left meth on counter. caps was called and they took my son away. in my police report the conclusion on child dangerment was denied when investigated but cps still has my son. they only know what the police told them at the time of the incident. apparently the higher ups in police denied the allegation. My husband owes child support arrearage for his youngest daughter for the first 2 years of her life. He has had sole custoday of the child since year 2 and modified the child support to start receiving support. The mother has not paid her support in over a year and now has accumulated arrearages. What is the procedure to get the courts and/or DES to offset her arrearage balance and future support against my husband's arrearage balance? At this point in time this will cut my husbands arrearage in half.

My Mother lived in and owned property in Sun City, AZ for about 30 yrs. She had two living children whom have had a rather toxic relationship. She had a living trust and her son is named Successor Trustee. In 2003 she suffered a small stroke and (while still mentally alert) she had a problem with her speaking ability. As a result, she retired as Trustee and named her son who divides his time between New Mexico and Alaska. Her son physically relocated her to New Mexico about 10 months before she passed in Feb. 2011. He had sold her property in AZ in Sept 2010 but moved all her personal belongings to his property in NM prior to that date. Her remains are interred in AZ. Which state would have jurisdiction if a probate proceeding is necessary? (i.e.- Say the Successor Trustee fails to act?)
