Question: I am a convicted felon, can I legally change my name?

Answer:

After receiving an application for name change, the court shall consider whether to grant the name change based upon prior felony convictions and the reason why the name change is requested. A.R.S. 12-601.A. & C.1.

Please see the form “Name Change for an Adult” for more information on what the court will request. This is the Maricopa County form. If another county’s form is needed, please use the court locator to find that county’s court.
