Question: I want to change my 3 year old daughter's name, but she does not have a father listed on her birth certificate. How do I go about this since there is not a father on her birth certificate?

Answer: You will find the information you need on the website for the Vital Records Section of the Arizona Department of Health Services.  The website contains information and detailed instructions for changing a child's name on a birth certificate.  The website www.azdhs.gov/licensing/vital-records/index.php.Because your child is more than one year old, you will need to obtain a court order from the Arizona Superior Court authorizing the name change.  You can do this on your own.  The superior court's website has forms and instructions.  Go to the Superior Court's website, find your county, and access the self-help center.  There you will find forms and instructions.  The website is www.azcourts.gov.
