The website contains information and detailed instructions for changing a child's name on a birth certificate. The website www.azdhs.gov/licensing/vital-records/index.php.Because your child is more than one year old, you will need to obtain a court order from the Arizona Superior Court authorizing the name change. You can do this on your own. The superior court's website has forms and instructions. Go to the Superior Court's website, find your county, and access the self-help center. There you will find forms and instructions. The website is www.azcourts.gov.
