Question: I would like to have assistance with filling out advance directives. Can you help?
Answer:
You should consult with an attorney. If you do not have one you can contact your local bar association for their lawyer referral service, which usually charge a small amount for a first consultation with a private attorney (usually $50 or less, but ask the referral service for the exact charge). If after the first consultation you want to have that attorney write up the directive for you, then make sure to have an agreement as to the cost. If you cannot afford an attorney, then call the agencies list on the home page.
A few things you may want to read before going to see an attonrey are reviewed in AZLawHelp Question 22085 Advanced directives which explains the written care instructions regarding your medical preferences. These directives will be used in the event that a person is unable to make their own health care decisions. http://www.jcl.com/hospitals/advance-directives.
