Answer: Name changes are handled by the Arizona Department of Health Services Vital Statistics Section. Because your child is over one year of age, a certified court order will be required. Go to https://www.azdhs.gov/licensing/vital-records/index.php#corrections-amendments-registrants-name and review the requirements. You can obtain the forms and instructions you need for the name change on the Arizona Court website. Go to: https://www.azcourts.gov/selfservicecenter/Forms/Name-Change .
