Question: I am the parent who receives the child support, I have remarried and have a different last name. is there a form that i need to fill out so the c.s. checks match that to my new bank account?
Answer: To change your name in the support system, you should contact the Clerk of the Superior Court for the county in which you live. The Clerk's office should be able to tell you what form you may need and give you instructions as to what to do. If you live in Maricopa County, you can find the information and form that the Maricopa County Superior Court uses online here.
