Question: I want to know if I change my name back to my birth name, if I can still use both names. i.e. Birth Name changed from Ernest (info removed), I want to continue using both names. i.e. My current legal name is Merrick (info removed)
Answer: Due to the application filled out for a name change, a person cannot go by their birth name and a current legal name. On the application to change your name, you must provide alias, as well as any criminal records that may have been filed. Go to http://www.azcourts.gov/Portals/34/Forms/NameChange/CVNC11fFinal.pdf to look at the form. Check with an attorney to see if there are any circumstances that would benefit you using both names legally.
