Question: My ex gave me a call 2 years ago telling me he is not my daughters father and wanted his name removed from her birth certificate. He has never been a part of her life and I have off idea where he is now how do I get about getting his arms removed from the birth certificate?

If you are outside of Maricopa County, go to: http://www.azcourts.gov/selfservicecenter/Self-Service-Forms for information on your county. You may also want to discuss your situation with an attorney.
