To start, you may want to go to the Superior court and see if they have a packet of forms and information on filing for visitation and custody. You can go to https://www.superiorcourt.maricopa.gov/SuperiorCourt/Self-ServiceCenter/Forms/FamilyCourt/index.asp to download the forms and information for Maricopa County. Pima County also has a self service desk with forms. If you are outside these counties, you may use these forms for your county. Follow the directions and take to the superior court to file. You may want to consult with an attorney for legal advice.
