Question: I have my 17 yr old disabled son his mother gave him to me so now he lives with me I'm not on the birth certificate Ineed to know how do iI get on his birth certificate I've already done paternity test for D.E.S and I pay child support.
Answer: First you would go to http://www.azdhs.gov/licensing/vital-records/index.php#birth-certificates-apply to apply for the birth certificate. You may need to contact them to find out what information they require since you have already done paternity. You may also need to contact the courts to have you listed as the father. You can go to http://www.azcourts.gov/selfservicecenter/Self-Service-Forms for the forms for your county.
