Question: Can two women sign the birth certificate in the state of Arizona if yes do they have to be married?
Answer: The Arizona Department of Health Services is responsible for vital records (>http://www.azdhs.gov/licensing/vital-records/index.php#birth-certificates-apply), to include birth certificates. If I understand Arizona Department of Health Services correctly, they do NOT allow more than one father or mother on the birth certificate. I would recommend you speak with a family law attorney, specifically an attorney who specializes in this type of situation.
