Question: I applied for birth certificate at hospital for my baby boy when he was born and I have not received it

Answer: Go online to the site for Arizona Department of Health Services.  You need vital records.  The site is:  www.azdhs.gov/licensing/vital-records/index.php#birth-certificates.  The site allows you to download the form to obtain the birth certificate and simple instructions telling you what to do.  You will need a photo ID like your driver license.  The fee for a certified copy of a birth certificate is $20 in both Maricopa and Pima counties.  You will need to check the website to find the fee for the other counties.
