Question: If I want to add my last name to birth certificate can I still do so ? Daughter is barely two weeks right now ! Is it to late ? How would I go about doing so ?
Answer: It is not too late. Amending or adding to a child's birth certificate is the task of the Arizona Department of Health Services Vital Records Section. Because your child is so young, you can add your name by affidavit. The Vital Records Department website has detailed instructions on what you need to do and how to do it. Start with the website. You may want to start the process without delay because the longer you wait, the more complicated the process becomes. You can also call and request an appointment.
