Question: I am a widow and would like to return to my maiden name; do I need to file a name change request? or can I just go back to my maiden name
Answer: You would need to file a name change with the Superior court in your county. If you are in Maricopa County, you can go to the self help service center to get the forms or go on line at: https://www.superiorcourt.maricopa.gov/SuperiorCourt/Self-ServiceCenter/Forms/CivilCases/civil_cvnca1.asp All the instructions and forms are there.
