Name Change
questions & answers
Question: I would like to change my last name. What would be the first thing I would need to do?
Answer: You will need to file a name change action in the Arizona Superior Court. It is a fairly simple process and should be granted so long as you are not changing your name for an illegal purpose. You can find the forms and instructions online at the Superior Court's website. Go to: https://www.azcourts.gov/selfservicecenter/Forms/Name-Change. The information is in the self-service center under civil law.
