Question: I had my daughter 3 years ago and did not have her father sign the birth certificate we are still together and he wants to sign her birth certificate how would we go about him signing it
Answer: Information about amending a birth certificate to add the father’s name and the necessary forms can be found here. http://www.azdhs.gov/vital-records/corrections-amendments/birth/paternity.htm The Arizona Department of Health Services Bureau of Health Status and Vital Statistics is the state department that records birth certificates. When both parents wish to acknowledge paternity, they must both complete and sign an Acknowledgement of Paternity form and the form must be either notarized or witnessed. Specific instructions can be found along with the appropriate form at the link above.
