Question: I want to know if it is possible to add another name to my baby of 6 months, for example if his name is Luis but I want his name to be Jose Luis

Answer: You can..  The Arizona Department of Health Services, the Vital Statistics Office is in charge of changing an infant's name.  The written instructions found on the website contain the following statement:  "If your child is over 3 months and less than 1 year of age, the parent or guardian shall submit the following to request the amendment:  a completed application, a valid government issued identification or notarized signature on the application, a completed affidavit, an evidentiary document created within one year after the date of the child's birth that includes the name to be added and the amendment fee."    Not all forms are accepted in all courts.  Please contact the Clerk of the Court in your county and ask exactly what forms you will need and what supporting documents are required.
