Question: How do you get the cost of a name change fee waived. I am on disability. Are there certain papers I need to show proof of income and if so which ones. How is a waive determined at the time of filing and who decides?
Answer: There is an article on AzLawHelp.org that give step by step instructions on name change. Go to: http://azlawhelp.org/articles_info.cfm?mc=1&sc=8&articleid=60 to read the article. There may be a form at the Superior Court's Self Service center that requests a waiver of fees for the name change. You would need to fill it out and the judge will decide if there is a reason to waive the fees. You can go to http://www.azcourts.gov/selfservicecenter/Self-Service-Forms for the forms and self service centers in your county.
