Question: My kids father abandoned us the day I out that I was pregnant th our 2nd child my son at the time was 1. He is now 7 and I'd like to remove the dad from my son's birth certificate. How can I do this?
Answer: In Arizona, a court order is required to remove a father’s name from a child’s birth certificate. When obtaining a court order, the order may be filed with the Office of Vital Registration directly by the Court, the agency that petitioned the Court for the order, or one of the parents may deliver it in person to one of the office locations of the Office of Vital Registration.
